What Are Mesenchymal Stem Cells? The Biology Behind the Therapy

📖 8 min read📅 Updated June 2026

Bottom Line Up Front

Mesenchymal stem cells (MSCs) are adult stem cells found in bone marrow, adipose (fat) tissue, and umbilical cord tissue. Contrary to popular belief, they work primarily through paracrine signaling (releasing beneficial molecules) rather than directly replacing damaged tissue. Understanding this mechanism is crucial for setting realistic expectations about what stem cell therapy can and cannot do.

The term “stem cell” conjures images of cells transforming into whatever tissue the body needs — damaged cartilage becoming new cartilage, injured nerves regenerating, aging organs rejuvenating. This is an oversimplification that leads to unrealistic expectations. Understanding what MSCs actually do helps you evaluate treatment claims more critically.

Types of Stem Cells

Embryonic stem cellsEarly-stage embryoCan become any cell type (pluripotent)Research only; ethical and regulatory restrictions

Mesenchymal stem cells (MSCs)Bone marrow, fat, umbilical cordLimited differentiation; primarily paracrine functionRegenerative medicine (Level 2–4 depending on condition)

How MSCs Actually Work

The dominant mechanism of action for MSCs is paracrine signaling. When injected into a joint, tissue, or infused intravenously, MSCs release a cocktail of anti-inflammatory cytokines (molecules that reduce inflammation), growth factors (molecules that support tissue repair), immunomodulatory signals (molecules that calm overactive immune responses), and angiogenic factors (molecules that promote blood vessel formation). These secreted molecules influence the surrounding tissue environment, promoting healing and reducing pathological processes. The MSCs themselves typically do not survive long-term at the injection site. They are biological pharmacies, not building blocks.
