Why steroids and biologics conflict
Intra-articular corticosteroids relieve pain by suppressing inflammation, and at joint concentrations they are directly toxic to chondrocytes (cartilage cells) and to mesenchymal stem cells in laboratory studies, reducing viability and the ability to differentiate. Repeated steroid injections are associated with faster cartilage loss on MRI in knee osteoarthritis trials. Injecting a cell product into a joint still carrying steroid, or into tissue whose repair signalling has been suppressed, is working against yourself.
How long clinics wait
| Interval since last steroid in the target joint | Typical clinic position |
|---|---|
| Under 4 weeks | Most decline to inject a biologic |
| 4 to 6 weeks | Minimum accepted by many protocols |
| 6 to 12 weeks | Common recommended window |
| 3 months or more | Preferred by conservative clinics, especially for expanded cells |
These intervals are consensus-based rather than derived from trials that varied the washout, so you will see different numbers. A clinic that will inject cells the week after your cortisone shot is not thinking about your outcome.
Oral and systemic steroids
A short prednisone burst for an unrelated condition is usually cleared within days, and most clinics want 2 weeks. Chronic oral steroid use requires an individual plan and is a relative contraindication to expanded-cell treatment at many clinics; see who clinics should reject.
Planning around a trip
If you are considering Colombia and your local doctor offers a cortisone shot to tide you over, understand that it may push your biologic injection back by two to three months. The rational sequence for someone committed to trying a biologic:
- Decline or defer further steroid injections in the target joint
- Get imaging and send it to the clinic for candidacy review
- Manage pain with acetaminophen, activity modification and physical therapy during the washout
- Schedule the trip for at least 6 weeks, ideally 3 months, after the last steroid
What to tell the clinic
Exact date, drug and dose of every injection into the target joint in the past year. Clinics also want to know about hyaluronic acid injections (less of a concern; 2 to 4 weeks is typical) and any previous PRP or cell injections. Put it in writing; see medications to stop before injection.
The bigger point
If cortisone has worked well for you, that is useful information: it suggests inflammation is a significant driver of your pain, and it is worth asking whether PRP, which is cheaper, would be the rational next step rather than expanded cells. See stem cells vs PRP vs cortisone.
Regulatory status: FDA vs INVIMA
United States (FDA): No stem cell product is FDA-approved for orthopedic, neurologic, or systemic use. Minimally manipulated same-day autologous procedures are marketed under a narrow exemption; culture-expanded cells and most umbilical products are unapproved biologics and the FDA's authority over them was reaffirmed in federal court in 2024. Florida's 2025 law permits certain treatments under state rules but does not create FDA approval.
Colombia (INVIMA): Cell therapies are regulated as advanced-therapy products. Clinics require INVIMA habilitation for the facility and, for laboratory-processed products, product-level authorization or an approved research protocol. Ask to see the INVIMA registration number and verify the treating physician on ReTHUS. "INVIMA registered" for the clinic is not the same as an authorized product.
Frequently asked questions
How long after cortisone can I get a stem cell injection?
Most clinics want at least 4 to 6 weeks; many prefer 3 months for the same joint.
Does one cortisone shot ruin a later stem cell treatment?
No. It is the timing and cumulative exposure that matter. A single shot followed by an adequate washout is fine.
What about hyaluronic acid injections?
Less of a conflict. A 2 to 4 week interval is typical.
Had a cortisone shot and planning a trip?
Tell us the date. We will work out the earliest sensible treatment window and which clinic protocols match it.
