The natural history first
Around 80 to 90 percent of plantar fasciitis resolves within 12 months with stretching, load management, footwear changes, night splints and, when needed, a short course of physical therapy. Chronic cases beyond 6 to 12 months are the ones that reach injection clinics.

Extracorporeal shockwave therapy (ESWT), which is not an injection, has L2 evidence and is widely available in Colombia at low cost. It belongs in the conversation before any needle.
Why stem cells are a poor fit here
The plantar fascia is a dense, relatively avascular band, and chronic fasciitis is a degenerative (fasciosis) rather than inflammatory process. Growth-factor stimulation, which PRP provides, has a plausible mechanism and trial support. Adding mesenchymal cells has no demonstrated advantage, costs five to ten times as much, and in the US puts the clinic on the wrong side of the FDA. See the orthobiologic ladder.
How this is marketed
Plantar fasciitis appears on nearly every "conditions we treat" list at stem cell clinics because it is common, painful and chronic. When you see it there alongside MS, autism and anti-aging, treat the whole list as a marketing document. A clinic that specializes in feet will offer ESWT and PRP first.
What to do in Colombia
- Podiatry or sports medicine evaluation, including ultrasound to measure fascia thickness and rule out a tear or nerve entrapment
- ESWT course (three to five sessions, typically USD 40 to 90 per session in Medellín or Bogotá)
- Ultrasound-guided PRP, one to two injections, USD 200 to 500 each, with a taping and loading program
- Surgery (partial plantar fascia release or gastrocnemius recession) for the small fraction that fail everything

Regulatory status: FDA vs INVIMA
United States (FDA): No stem cell product is FDA-approved for orthopedic, neurologic, or systemic use. Minimally manipulated same-day autologous procedures are marketed under a narrow exemption; culture-expanded cells and most umbilical products are unapproved biologics and the FDA's authority over them was reaffirmed in federal court in 2024. Florida's 2025 law permits certain treatments under state rules but does not create FDA approval.
Colombia (INVIMA): Cell therapies are regulated as advanced-therapy products. Clinics require INVIMA habilitation for the facility and, for laboratory-processed products, product-level authorization or an approved research protocol. Ask to see the INVIMA registration number and verify the treating physician on ReTHUS. "INVIMA registered" for the clinic is not the same as an authorized product.

Do stem cell injections cure plantar fasciitis?
