How often blind injections miss
Studies comparing landmark-guided (blind) injection with imaging-guided injection have measured accuracy by contrast or by direct verification. Reported blind-injection accuracy varies widely by joint and operator: the knee is most forgiving, with blind accuracy commonly reported around 70 to 90 percent depending on approach; the hip and shoulder are far worse, with blind hip injections landing outside the joint in a large minority of attempts. Guidance raises accuracy to the high 90s across joints.
For a steroid injection that costs USD 100, a miss is an inconvenience. For a cell or PRP product, a miss wastes the product and produces a false negative that convinces the patient the therapy "did not work."

What guidance also tells you about the clinic
- The physician knows anatomy well enough to interpret an image in real time
- The clinic has invested in equipment rather than only in marketing
- There is a record: guided injections produce saved images that go in your file, useful if there is ever a dispute about what was done
Questions to ask
- "Is every injection done under ultrasound or fluoroscopy?" The answer should be yes, without exceptions for "easy" joints.
- "Who performs the injection: the physician, or a nurse or technician?" Cell injections should be physician-performed.
- "Can I have the images?" You should be able to.
- "Do you inject through the same needle used for harvest?" No; separate sterile technique for harvest and injection.
What a good injection visit looks like in Colombia
Pre-procedure imaging review, skin preparation with chlorhexidine, sterile probe cover, local anesthetic (without steroid), needle visualized entering the joint or tendon sheath, the product delivered slowly with the needle tip on screen, and a saved image with your name and date. Regulatory status: FDA vs INVIMA
United States (FDA): No stem cell product is FDA-approved for orthopedic, neurologic, or systemic use. Minimally manipulated same-day autologous procedures are marketed under a narrow exemption; culture-expanded cells and most umbilical products are unapproved biologics and the FDA's authority over them was reaffirmed in federal court in 2024. Florida's 2025 law permits certain treatments under state rules but does not create FDA approval.
Colombia (INVIMA): Cell therapies are regulated as advanced-therapy products. Clinics require INVIMA habilitation for the facility and, for laboratory-processed products, product-level authorization or an approved research protocol. Ask to see the INVIMA registration number and verify the treating physician on ReTHUS. "INVIMA registered" for the clinic is not the same as an authorized product.
Frequently asked questions
Is ultrasound guidance really necessary for a knee injection?
Knee blind accuracy is better than other joints but still imperfect, and the product being injected is expensive. For cell and PRP products the standard is guidance for every joint.
Does guidance add cost?
Marginally. In Colombia it is typically included; a clinic charging a large surcharge for ultrasound guidance is unusual.
Can a bad injection technique cause harm?
Yes: infection, bleeding, nerve or vessel injury, and cell product placed in the wrong tissue. Guidance reduces all of these.
